Who Qualifies

Our program serves specific populations facing housing challenges. We help connect individuals and families with appropriate pathways.

Parents Needing Housing

Families with children facing housing instability who need structured guidance and coordination toward stable housing.

  • At least one child in household
  • Currently housed but at risk, OR
  • Currently unhoused

Adults Needing Reset

Adults who need a structured pathway to regain housing stability after disruption or transition.

  • Adults 18+
  • Ambulatory (able to move independently)
  • Ready to engage with process

Discharge Situations

Individuals being discharged from hospitals or care facilities who need coordinated housing placement.

  • Ambulatory only (no wheelchairs)
  • Medically stable for discharge
  • Coordinating facility involved

Important: We do not provide emergency shelter. We coordinate with existing housing resources and pathways.
